Community Empowerment Resources (CER)
Join the team at Community Empowerment Resources (CER) and make a meaningful difference in the lives of adults living with serious and persistent mental illness (SPMI). We are seeking a compassionate, motivated, and recovery-oriented Community-Based Case Manager to provide individualized support to clients in the community.
CER is a mission-driven nonprofit behavioral health organization dedicated to providing inclusive, comprehensive services to individuals experiencing mental illness, co-occurring substance use disorders, and housing instability. Our approach emphasizes person-centered care, trauma-informed practices, and empowering individuals to build stability, strengthen recovery, and work toward greater independence.
As a Community-Based Case Manager, you will build trusting, clinically informed relationships with clients and help them navigate complex behavioral health, housing, medical, and social needs. Through individualized recovery planning, service coordination, and collaboration with providers, families, and community partners, you will support clients in achieving their personal goals and improving their overall quality of life.
At CER, we believe meaningful relationships are essential to effective care. Our intentionally low caseloads of approximately 12–16 clients allow you to dedicate more time to each individual, foster consistent engagement, and provide high-quality, individualized support.
If you are passionate about mental health recovery, community-based services, and helping individuals move toward greater stability and independence with dignity, we invite you to join our team and be part of a mission that makes a lasting impact.
Responsibilities
- Develop, implement, and update individualized service and recovery plans based on each client’s goals, strengths, needs, and preferences.
- Build and maintain trusting, therapeutic, and recovery-oriented relationships with clients that promote engagement, stability, and independence.
- Coordinate and facilitate access to psychiatric, medical, behavioral health, housing, legal, vocational, substance use, and other community-based services.
- Provide consistent, person-centered support through home visits and community-based meetings to address client needs and support progress toward recovery goals.
- Collaborate with treatment providers, families, and community partners to coordinate care, address barriers, and promote continuity of services.
- Provide transportation and accompany clients to appointments or community resources when clinically appropriate and safe.
- Monitor client progress, identify emerging needs or barriers, and adjust service plans in collaboration with clients and the treatment team.
