Health Concierge - Clinical Scheduling Coordinator

Across Home Health, we’re transforming the way we work in our local communities to improve health and care outcomes for customers. By working together, differently – we believe we will be able to plan and deliver better care for customers.

The Health Concierge – Clinical Team provides personalised and streamlined best practice clinical service triaging, prioritisation, clinical referral review and escalations, and scheduling experience across multiple programs of care, recovery, and treatment. The aim of this function is to match the right resource, to the right need, at the right time, to deliver the best possible outcome.

Your opportunity

As a Clinical Scheduling Coordinator, you’ll play a pivotal role in ensuring our customers receive timely, high-quality care. In this fast-paced role, you’ll manage the scheduling of allied health and nursing services, triage referrals, coordinate bookings, and respond to intraday changes. You’ll be the first point of contact for service changes, supporting both customers and clinicians by maintaining stable, responsive schedules. Working closely with workforce planning and operations teams, you’ll help deliver seamless care experiences. Your coordination ensures our customers can continue living independently at home, while enabling our clinical workforce to operate efficiently and compassionately.
