Lead Licensed Physical Therapist

DIRECTOR OF REHABILITATION — PHYSICAL THERAPY

Premier Senior Living Community | Frederick, MarylandWhat if your rehab program could be part of the way a community helps residents live—not just the place they go when they need therapy?

We're looking for a Physical Therapist and clinical leader who sees senior living differently.

This is a community built around movement, engagement, independence, and exceptional experiences. Residents participate in fitness, social programs, outings, dining, education, and daily life on their terms. The community also supports Assisted Living, Memory Care, restorative care, and specialized Parkinson's programming.

That creates an incredible environment for a PT who understands that mobility is about much more than gait and strength.

It's about getting someone back to the dining room.

Walking confidently to an activity.

Staying independent in their apartment.

Returning to something they love.

Helping a resident with Parkinson's move with greater confidence.

Recognizing a change before it becomes a crisis.

THIS IS CLINICAL LEADERSHIP.

As Director of Rehabilitation, you'll lead the physical therapy program while remaining clinically engaged. You'll develop your team, elevate clinical reasoning, build relationships across the community, and partner with leadership to make rehabilitation an integral part of the resident experience.

You'll:

• Lead and develop an exceptional therapy team

• Maintain an active clinical caseload

• Drive clinical quality, outcomes, and evidence-informed practice

• Build strong relationships with residents, families, caregivers, nursing, and community leadership

• Develop programming around mobility, balance, fall prevention, strength, function, and independence

• Support residents across Assisted Living, Memory Care, and wellness programming

• Identify opportunities for early intervention and proactive clinical support

• Build strong referral and interdisciplinary relationships

• Use data and clinical insight to continually improve the program

And you'll have Prava behind you.

Prava provides the technology, clinical infrastructure, documentation tools, RCM, credentialing, operational support, and resources that allow great clinicians to focus on leading great care.

**You bring clinical leadership.
We bring the operating system.
Together, we elevate what rehabilitation can look like in senior living.**

If you're ready to lead a program where therapy is connected to real life—and where your clinical voice actually matters—we'd love to meet you.
