Licensed Occupational Therapist - PRN

PRN OCCUPATIONAL THERAPISTMission Commons | Redlands, California

Prava Therapy**

Great occupational therapy doesn't just help someone do more.It helps them keep doing what makes life meaningful.

Prava Therapy is looking for a PRN Occupational Therapist to join our embedded therapy team at Mission Commons.

This isn't traditional senior living therapy.

You're not walking into a facility, seeing a list of residents, completing your visits, and heading home.

You're becoming part of a community.

You'll know the residents.

You'll know the team.

You'll understand the rhythms of the community—and you'll use that knowledge to make therapy more meaningful, more functional, and more connected to everyday life.

THIS IS OT WITH A BIGGER PURPOSE.

Occupational therapy is about much more than an exercise program.

It's about helping someone safely return to the dining room.

Manage their morning routine.

Participate in an activity they love.

Navigate their apartment.

Engage with friends.

Communicate with their family.

Stay independent in the things that matter most.

Sometimes it's improving strength or balance.

Sometimes it's cognition.

Sometimes it's adapting the environment.

Sometimes it's teaching a caregiver a different way to support a resident.

And sometimes it's simply helping someone rediscover confidence in something they thought they couldn't do anymore.

That's the work.

YOUR ROLE

As a PRN OT with Prava, you'll provide skilled occupational therapy while becoming a trusted clinical presence within the community.

You'll work alongside residents, families, caregivers, nursing, and community leadership to help residents remain as independent, engaged, and connected as possible.

You'll:

• Provide exceptional, individualized occupational therapy based on each resident's goals, needs, interests, and daily routines.

• Evaluate and treat residents across a range of functional, cognitive, and physical needs.

• Develop meaningful plans of care focused on participation, independence, safety, and quality of life.

• Address ADLs, IADLs, cognition, balance, functional mobility, upper-extremity function, and environmental barriers as appropriate.

• Help residents participate in the activities and routines that give their days meaning.

• Recognize changes in function and communicate concerns proactively with the interdisciplinary team.

• Partner with caregivers and community staff to reinforce strategies beyond the therapy session.

• Provide education that helps the people around the resident support greater independence.

• Build strong relationships with residents and families.

• Collaborate with the broader Prava therapy team and community care team.

• Document thoughtfully and efficiently using Prava's technology and AI-enabled tools.

• Measure outcomes that matter—not simply minutes or visits.

• Help create a therapy culture that is visible, responsive, and integrated into the community.

WE'RE LOOKING FOR AN OT WHO SEES THE WHOLE PERSON.

You don't see a diagnosis and immediately reach for a protocol.

You get curious.

What does this resident want to do?What's getting in the way? What can we change? How can we make success more likely?

You believe:

Every intervention should have a functional purpose.Occupation matters. The environment can be part of the solution.Residents should be appropriately challenged—not underestimated.Caregiver education is part of treatment.Outcomes matter more than minutes.Dignity and autonomy are clinical priorities.

And you understand that sometimes the most meaningful clinical intervention happens outside the therapy space.

YOU'LL HAVE A COMMUNITY — NOT JUST A CASELOAD.

At Kensington Sierra Madre, you'll have the opportunity to become familiar with the people you're serving and the environment in which they live.

You'll see residents in the dining room.

In the hallway.

At activities.

With their families.

You'll notice the little things.

A resident who isn't getting around quite as easily.

Someone struggling with a routine they used to manage independently.

A caregiver who needs a new strategy.

A change that might warrant intervention before it becomes a bigger problem.

That's the advantage of being embedded.

You don't just treat the resident.

You understand their life.

YOUR CLINICAL EXPERTISE + PRAVA'S OPERATING SYSTEM

You bring the clinical expertise.

We bring the infrastructure.

Prava provides the technology, clinical standards, AI-enabled documentation, credentialing, billing and revenue cycle support, and operational systems that help clinicians focus on exceptional resident care.

You get the support of a modern therapy organization without losing the autonomy and connection that make senior living therapy so rewarding.

THIS ROLE IS FOR YOU IF...

• You're an OT who loves what occupational therapy is really about.

• You see occupation—not diagnosis—as the heart of your work.

• You enjoy building genuine relationships with residents and families.

• You're comfortable working across disciplines and partnering with caregivers.

• You're curious, creative, and willing to think beyond traditional treatment approaches.

• You enjoy senior living and believe residents deserve therapy that respects the lives they've built.

• You want to work somewhere your clinical judgment is valued.

• You want to be part of a therapy model that is modern, technology-enabled, and deeply human.

• You want your work to have an impact beyond the treatment session.

WHAT SUCCESS LOOKS LIKE

Residents trust you.

Families value your perspective.

Caregivers have practical strategies they can use.

Community leadership sees you as a clinical partner.

Your interventions translate into real life.

And residents are able to keep participating in the routines, relationships, and experiences that make their lives meaningful.

That's occupational therapy at its best.

READY TO PRACTICE OT DIFFERENTLY?

If you believe occupational therapy should be about more than treating impairments—

we think you'll fit right in.

Bring your clinical expertise.

Bring your creativity.

Bring your curiosity.

We'll bring the operating system.

Come help residents keep living the lives they love.
