Per Diem Occupational Therapist - Norwalk, CT

Constellation Home Care is seeking a Per Diem Occupational Therapist (OT) to work with our home care team in Norwalk, CT!Flexible Hours! Newly licensed OTs are encouraged to apply!

This is a field-based position, requiring the Occupational Therapist to develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions. You will offer effective and client-centered services that will enable people to carry out everyday tasks or activities confidently and independently.

Responsibilities:

• Provide exceptional Occupational Therapy (OT) to patients by assessing and understanding the care plan.

• Evaluate patient's functional status and home environment for hazards or barriers to be independent living and identify any equipment needs.

• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.

• Participate in the development of the individualized plan of care.

• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.

• Submits all clinical documentation, time records and other information in the required time frame.
